As artificial intelligence becomes embedded in critical operations, the challenge shifts from building capable systems to ensuring they are governed responsibly across their entire lifecycle. ISO/IEC 42001:2023, the first international management system standard dedicated to AI, establishes a structured framework for governance that integrates ethical, technical, and compliance considerations from inception to retirement.

AI governance, as defined in the standard, encompasses organizational structures, policies, and controls designed to ensure AI is used ethically, safely, and in alignment with stakeholder expectations. This spans activities such as defining intended purpose, managing data and model risks, embedding explainability and bias mitigation, and maintaining accountability through monitoring and decommissioning.
The lifecycle perspective is critical. ISO/IEC 22989:2022 outlines seven stages: inception, design and development, verification and validation, deployment, operation and monitoring, re-evaluation, and retirement. Each stage presents distinct risk profiles. For example, inception may involve spoofing threats, while operation and monitoring must guard against denial-of-service attacks. ISO/IEC 42001 links these stages to specific clauses and Annex A controls, ensuring risks are addressed systematically.
Risk management under ISO/IEC 42001 begins with identifying and assessing AI risks (Clause 6.1), followed by implementing operational controls (Clause 8.2) and maintaining continuous monitoring and improvement (Clauses 9 and 10). In high-impact scenarios, the standard calls for AI impact assessments (AIIAs), which focus on societal, ethical, and legal implications. These assessments parallel data protection impact assessments (DPIAs) under privacy laws such as the GDPR, but extend beyond personal data to consider fairness, discrimination, and proportionality.
Organizations can select methodologies suited to their context. ISO 31000 offers a general enterprise risk management approach, while the NIST AI Risk Management Framework provides AI-specific guidance structured around the functions Map, Measure, Manage, and Govern. At the technical layer, threat modeling tools such as STRIDE, DREAD, and OWASP for machine learning enable detailed analysis of vulnerabilities, adversarial risks, and privacy threats.
The standard’s tiered governance model integrates these layers: ISO/IEC 42001 at the top for strategic oversight, widely adopted risk frameworks in the middle, and granular threat modeling at the base. This alignment ensures that high-level governance translates into concrete technical safeguards.
Threat modeling plays a pivotal role in this ecosystem. STRIDE, for instance, categorizes threats into spoofing, tampering, repudiation, information disclosure, denial of service, and elevation of privilege. Applied across the AI lifecycle, it reveals stage-specific vulnerabilities: tampering risks during design, repudiation issues in verification, or privilege escalation during re-evaluation. Other methods, such as PASTA or LINDDUN, can be employed depending on system context.
AIIAs are particularly important for AI deployed in sensitive domains like healthcare, finance, or public services. They document the system’s purpose, map affected stakeholders, evaluate legal and social risks, and propose mitigation and oversight mechanisms. The outcome should be a transparent record of potential impacts and the measures in place to address them, with clear governance responsibilities and triggers for reassessment.
Once risks are identified through threat modeling and impact assessments, they must be mapped to ISO/IEC 42001 controls. Progress and risk metrics should be reported to top leadership, and incidents analyzed to drive system improvements.
