Air India 787 Engine Shutdown Led GE to Widen Safety Inspections
A compressor blade came loose inside an Air India Boeing 787-8 engine during flight, causing a surge and shutdown that forced the aircraft to return to Mumbai on one engine. India’s Directorate General of Civil Aviation has now disclosed that the 2023 incident prompted GE Aerospace to widen internal inspections to additional affected engine serial numbers. The regulator said the resulting inspection requirements were completed on time.

The incident began during the climb from Mumbai on August 4, 2023. Air India flight AI-131, aircraft registration VT-ANW, was headed to London Heathrow with 267 people aboard when Engine No. 1 surged and then shut down. The crew returned to Mumbai and completed an overweight, single-engine landing. No injuries were reported.
The final investigation report traced the shutdown to a high-pressure compressor Stage-10 blade that was released because its locking lugs had been installed improperly during maintenance in 2018. Investigators connected that blade release to the surge and subsequent engine shutdown. The finding establishes a documented maintenance-related cause for this particular event, but it does not support broader conclusions about every Boeing 787, every GE engine or unrelated aviation incidents.
GE expanded the serial-number inspection scope
GE Aerospace issued service bulletin SB 72-0543 on December 21, 2023. According to the DGCA’s account to the Central Information Commission, the bulletin expanded borescope inspection requirements for affected engines and included additional engine serial numbers. A borescope allows maintenance personnel to inspect internal engine areas without a complete teardown, making it a practical way to look for specified conditions across an identified population.
The move from one investigated failure to a broader, serial-number-based inspection scope is the central continued-airworthiness response. It indicates that GE did not limit the follow-up to the engine involved in the flight. At the same time, the available information does not identify how many engines were added, which operators had affected units or what inspectors found. It therefore would be inaccurate to describe the action as a fleet-wide inspection of all 787s or to assign a numerical scale to it.
Serial-number applicability is an important boundary in engine safety work. Commercial aircraft of the same model can have different engine configurations, production histories and maintenance records. A service bulletin can define the equipment requiring attention more precisely than a blanket aircraft-model label. In this case, the DGCA described GE’s action as covering “affected engines” and additional serial numbers, not the entire global Boeing 787 fleet.
Completed compliance does not fill every information gap
The DGCA said compliance with the bulletin “was completed within the prescribed timeline.” Its Directorate of Airworthiness consequently treated the recommended corrective action as duly complied with. That confirms completion of the required inspection response under the regulator’s account. It does not, on the information made public, establish the number of engines inspected, whether any additional discrepancies were discovered or whether parts were removed from service.
The distinction matters for passengers and operators. The immediate operational consequence in 2023 was substantial: a long-haul flight carrying 267 people had to turn back, operate with one engine and land above its normal planned landing weight. The aircraft nevertheless landed safely, and no injuries were recorded. The subsequent inspection expansion addressed continued operation of the specifically affected engine population rather than suggesting that the safe landing erased the underlying maintenance concern.
The disclosed chronology also illustrates the long reach of maintenance quality. The locking-lug installation occurred in 2018, while the blade release and shutdown happened five years later. The investigation finding links those two events in this case, although the maintenance organization responsible for the installation has not been identified publicly in the available account. Neither GE Aerospace nor Air India responded to queries associated with the disclosure.
The details emerged through a Right to Information proceeding after the Central Information Commission found that the DGCA’s initial response did not adequately address what action had been taken following the investigation. The commission later accepted that the regulator had furnished the action records available to it and directed the DGCA to provide its written submissions to the applicant and file a compliance report. That process produced the key new fact: GE’s response extended beyond the failed engine to additional serial numbers, and the regulator considers the resulting inspection action complete.
